PLEASE READ CAREFULLY BEFORE DOWNLOADING FORMS BELOW
If you are a minor dead-set on getting Pierced or Tattooed, please share your desire with your parent or guardian. Tattooing and Piercing have become more mainstream. Your parents may be more open to the idea if you approach them with knowledgeable information.
REMEMBER WE ARE NAMED TATTOOS FOREVER, NOT TATTOOS UNTIL YOU CHANGE YOUR MIND...
CONSENT FORMS
We are parents, brothers, sisters and adults too. We recognize the fact that under- age kids who want to be pierced and/or tattooed, even without a parents consent, may go to extremes to fulfill this desire. If your child is open with you and shares such feelings, we would much rather, with your consent of course, your child be tattooed in a friendly and safe environment. We as are professionals trained to work with parents and their minor children.
Please note:
PIERCING AND TATTOOING
The necessary forms must be completely filled out and notarized
Minors under the are of 17 must be accompanied by a parent or legal guardian
Please be prepared to present your ID(s)
PIERCING
If you are a Minor who has parental approval and follows above criteria you may be Pierced at the Fort Walton Beach or the Crestview Studio.
TATTOOING
If you are a Minor who has parental approval and follows above criteria you must come to the Crestview Studio for a tattoo. This is a zoning issue and there are no exceptions.
